If your dog is microchipped and you indicate this when completing the online form, you will need to attach a copy of your dog's Microchip Certificate. Please ensure you have this document scanned and ready to attach (preferably in PDF or JPG format). If you have a smartphone, you can take a photograph of the certificate and upload it as a JPG file.

If you indicate that your dog has been de-sexed, you can also upload a scanned copy of the de-sex certificate in this online form. If you do not upload a copy at this time, you will be contacted to provide a copy when this registration is complete.
